Procedures for the Disclosure, etc. of Matters Related to Retained Personal Data Pursuant to the Personal Information Protection Act

Follow the procedures below if you wish to request notification, disclosure, correction, and suspension of use, etc. of matters related to retained personal data pursuant to the Personal Information Protection Act.

What to Prepare

1.When a request is made by an individual on his/her own behalf

Prepare the following documents and send them to “Contact point for disclosure requests.”

  • (1)Request form for disclosure, etc. of retained personal data
    Enter information within the bold frame and affix your registered seal.

  • (2)Document for confirming the identity of the individual
    One of the following documents:
    • Your seal registration certificate (within the last 6 months and with your current address)
    • A copy of a document issued by a public institution that can confirm your address, such as a driver's license, passport, health insurance card*, pension book, etc.
    • *For a copy of the health insurance card, mask the insurer number and insured person symbol/number.
  • (3)JPY1,000 in postage stamps
    This is the fee for responding to the "Retained Personal Data Disclosure, etc. Request Form” pursuant to the Personal Information Protection Act
    If our response does not contain personal information, we may refund the fee.
  • (Note 1)Please make sure that the document for confirming the identity of the individual contains your current address before submitting it.

2.When the request is made by a representative

Prepare the following document(s) in addition to 1 above.

  • (1)Document(s) for confirming the identity of the representative
  • If the representative is a person with parental authority "Family register copy" showing the relationship to the individual
    If the representative is an adult guardian The “written ruling for commencement of guardianship” or “Certificate of Matters Registered for Adult Guardianship”
    If the representative is an authorized agent “Power of Attorney” for making a proxy request on behalf of the individual concerned(with the individual’s registered seal affixed, photocopies not accepted) and the individual's ‘Seal Registration Certificate’ (within the last 6 months)
  • (2)Document for confirming the identity of the representative

    The seal registration certificate of the representative (within the last 6 months and with your current address) or a copy of a document issued by a public institution that can confirm your address, such as a driver's license, passport, health insurance card*, pension book, etc.

    • *For a copy of the health insurance card, mask the insurer number and insured person symbol/number.
  • (Note 1)If the request is made by a representative, the “Retained Personal Data Disclosure, etc. Request Form” pursuant to the Personal Information Protection Act
  • (Note 2)Please create the “Power of Attorney” with a letter of proxy, the name and address of the person authorized, and the individual’s own registered seal affixed. In addition, please include a statement to the effect that you entrust requests for disclosure, etc., to the representative as well as whether or not sensitive information can be disclosed to the representative.
  • (Note 3)If the representative is a person with parental authority or an adult guardian, your seal is not required in the identity column of the “Retained Personal Data Disclosure, etc. Request Form” pursuant to the Personal Information Protection Act
  • (Note 4)Please make sure that the current address of the representative is listed before submitting the document for confirming the identity of the representative.

Requests Regarding Procedures

1.Costs to be borne by the Customer's for procedures

Note that the customer will bear the following costs.

  • (1)Issuance fee for “Seal Registration Certificate” and other documents required for the customer’s request
  • (2)Costs related to the mailing of documents from the customer to the Company
  • (3)A fee for responding to the customer’s requested items (JPY1,000 in postage stamps)
    For details, refer to 1.(3)of “What to Prepare” above.

2.Time required for response

Please allow one week to one month for a response due to administrative procedures, confirmation, and internal investigations.

3.Handling of any deficiency in administrative procedures or billing documents, etc.

If there are any deficiencies in administrative procedures or billing documents, we will return the documents with the details of the deficiencies.

Note that we are unable to disclose any retained personal data that falls under the following categories:

  • Where the disclosure may harm the life, body, property, or other rights or interests of the individual concerned or a third party
  • Where there is a risk of significant hindrance to the proper implementation of our business due to the disclosure etc.
  • Where the disclosure would violate other laws or regulations

Note that if it is difficult to disclose information using the method nominated by the customer’s, we may have to disclose the information in writing.
